Blue and Yellow Forms
School forms 680/3040 formerly known as the "blue and yellow" forms are no longer required to be colored copied. These forms are only necessary for Kindergarten entrance, 7th grade school year and if your child is transferring from a county to county school.

Patient Centered Medical Home
According to Wikipedia, Patient Centered Medical Home (often referred to as PCMH) can be defined as such: " PCMH is a team based health care delivery model led by a physician that provides comprehensive and continuous medical care to patients with the goal of obtaining maximized health outcomes." It is the new approach on patient quality care.
